Madeline Jacobs is a trusted strategic advisor to healthcare and social service organizations, with more than two decades of experience improving programs, strengthening outcomes, and advancing equity across aging services, palliative care, home and community-based supports, and complex care delivery.
Known for her ability to bridge strategy and practice, Madeline helps nonprofits, foundations, and health systems design scalable programs, adopt innovative care models, and lead organizational change from within. Her work spans strategic planning, stakeholder engagement, and implementation of culturally responsive, person-centered care.
She has held leadership roles at the Center to Advance Palliative Care, Selfhelp Community Services, United Hospital Fund, and the Metropolitan Jewish Health System. Across these positions, she has led efforts to improve care transitions, expand access for underserved populations, and transform approaches to serious illness care.
Madeline holds an MPA from NYU’s Wagner School of Public Service and a BA from Rutgers University. Based in Brooklyn, she is an active community member and serves on the board of a local senior center.
